Mostly from shooting in low light. Raising ISO amplifies the sensor’s signal and its random error along with it, producing the speckled grain you see in dim indoor and night photos. Small sensors — phones and compacts — show it sooner than larger ones because each pixel gathers less light.
Because noise and fine detail look similar to a filter. Removing one inevitably takes some of the other, which is why heavily denoised photos develop that waxy, plastic look — particularly on skin and foliage. Use the lightest setting that makes the noise acceptable rather than the strongest available.
Before sharpening, always — sharpening amplifies noise, so doing it first makes the grain far more obvious and harder to remove afterwards. Denoise first, then sharpen gently, then resize. Getting that order wrong is why some edited photos look worse than the originals.
